What's the Project?
The client is a large US-based company from the healthcare market. We are starting a multi-phase, 2 years project focusing on replacement of the clinical and data platform, replacing the current system. The work will be concentrated on delivering a flexible and configurable workflow solution. You will work in a mid-size team (20-40 people), predominantly based in Central Europe, with a client in the United States. This will be an exciting, greenfield project in a highly demanding but also rewarding environment of modern healthcare, based on cloud architectures, in data-driven and AI-enhanced environment
Role Overview:
We are seeking an experienced Senior to Lead-level Backend Engineer to architect and develop a sophisticated Utilization Management and Prior Authorization application using the latest .NET frameworks. This role requires deep expertise in building scalable, event-driven architectures with complex workflow orchestration capabilities to support healthcare authorization workflows and clinical decision support.
You’re a perfect match if you have:
- 5+ years of professional experience building backend services / APIs, with at least 2+ years working with Nest.js in production.
- Deep proficiency in TypeScript; solid understanding of JavaScript core.
- Strong design skills in API architecture: REST + possibly GraphQL / WebSocket where needed.
- Experience with SQL and/or NoSQL databases; schema design, query optimization, migrations.
- Experience building modular systems / microservices.
- Thorough experience with testing (unit, integration, mocks, error paths).
- Strong skills in version control (Git), code reviews, and CI/CD pipelines.
- Good understanding of cloud deployments (e.g., AWS, Azure, or comparable) and shipping services to production with monitoring, logging, alerting.
- Familiarity with containerization (Docker) and orchestration is a plus.
- Excellent problem-solving skills, ability to debug complex issues in production.
- Strong verbal and written communication skills; ability to articulate trade-offs, document work.
Technical Leadership (only for Lead candidates): - Proven ability to mentor and guide development teams
- Experience conducting code reviews and establishing coding standards
- Track record of making architectural decisions for enterprise systems
- Ability to evaluate and recommend new technologies and approaches
- Strong documentation and communication skills
Nice to Have:
- Previous track record with US-based client
- Understanding of healthcare domain
- Microsoft certifications (Azure Developer Associate, Azure Solutions Architect)
- Experience with distributed tracing and observability (Application Insights, Seq, ELK)
- Knowledge of performance profiling and optimization tools
- Contributions to open-source projects
- Speaking experience at technical conferences or meetups
Your day-to-day activities
- Design and develop high-performance backend services using .NET 8+ and ASP.NET Core
- Architect and implement state-driven workflow engines with rules-based configuration systems
- Build and maintain event-driven architectures using message bus patterns (Azure Service Bus, RabbitMQ, or similar)
- Develop and optimize complex database solutions using MS SQL Server.
- Design microservices architectures with proper service boundaries and inter-service communication patterns
- Participate in implementing CI/CD pipelines and maintain code quality standards in Azure DevOps (ADO)
- Collaborate with cross-functional teams to deliver scalable, maintainable solutions
